information about you and your health. We
create a record of the care and services you
receive from us. We need this record to
provide you with quality care and to comply
with certain legal requirements. This notice
applies to all of our records.
We are legally
required to protect the privacy of your
health information. We call this information
"protected health information" or "PHI" for
short. PHI includes information that can be
used to identify you that we have created or
received about your past, present, or future
health or condition and related health care
services. We must provide you with this
notice about our privacy practices that
explains how, when, and why we use and
disclose your PHI. With some exceptions, we
may not use or disclose any more of your PHI
than is necessary to accomplish the purpose
of the use or disclosure. We are legally
required to abide by the privacy practices

to the health information that you may give
us, we may collect health information about
you from other sources, such as your family,
your legal representative, insurance
companies, other health care providers, and
