How can I set up a visit to get medical care?
Call any of our health centers. A staff member will be glad to help you. Click here for our phone numbers and a map.
   
   
What do I need to bring for my medical visit?
1. Bring your insurance card to every visit.
2. Bring ALL of your medicine on each visit (even aspirin or other medicine).
3. Bring a list of things you wish to discuss with the person caring for you.
4. New patients must bring a driver's license or other picture I.D., a water or light bill with their name and address; check stubs for one month's work. The center also needs to know how much money each person in your family earns. Persons not working must bring a letter from the Employment Security office stating they are not working.
5. Low income patients might be able to get help with medical bills and medicine.
   
NOTE: Items 1, 2, 3, 4 and 5 are used to help us compute the fees for our services. Until we get items 1, 2, 3, 4, and 5, you and your family members must pay the full cost for our service.
   
   
When I need medical care fast what should I do?
Call 911 in an emergency. An ambulance will be sent to your location, if needed.
   
   
When I am ill and the center is closed, what can I do?
Call 911 in an emergency. If you need to talk to a doctor and our office is closed please call (843) 248-1915. The medical person on duty will return your call.
   
   
What types of insurance do you accept?
We accept Medicare and Medicaid, and most major health insurance.
   
   
Can I get medical care if I do not have insurance?
Yes. Insurance is not needed to get medical care at our centers. We count the number of people living in the house and what each person earns to determine your fee.
   
   
What happens when I cannot pay my bill?
We expect you to pay after each medical visit. We will set up a pay plan for you if you are unable to pay. In order to set up a visit with your doctor you must pay your fees on time.

Urgent care patients visiting the center, who cannot pay, are checked by our nurse. Our nurse decides if a patient who claims to be sick, but cannot pay, will see a doctor that day.

When I call no one answers the phone; my call is not returned; I am placed on hold; or sent to the wrong person. How can I avoid being placed on hold or sent to the wrong person?
Please keep trying. Health Care Partners gets more than 1,900 calls each day. We know our phone system has problems. We are working to upgrade the phone system. Please listen carefully to the menu. Also, please leave just one message. We want to answer each call by the end of the day. If your call is urgent please hang up and dial 911.
   
   
How do I get my medicine refilled?
You can dial 249-2400 ext. 5376 for our refill hotline. Please give your name, date of birth, the name of your medicine, and a phone number where you can be reached.

If you do not have refills left, call your nurse. Please give your name, date of birth, the name of your medicine and a phone number where you can be reached. The nurse also needs the phone number of your pharmacy.

PLEASE REMEMBER! IT TAKES 48 HOURS TO REFILL YOUR MEDICINE. SO, ONE WEEK BEFORE YOU USE ALL YOUR MEDICINE, PLEASE CALL OUR REFILL HOTLINE!

If you are not happy with our services, please write our Chief Executive Officer. For nursing issues please call 248-4700 ext. 6034, or call 248-4700 ext. 6032 to set up a visit with the Chief Executive Officer.

The center needs volunteer help with clerical work. Spanish-speaking volunteers are also needed. The Reach Out and Read program, gives people of all ages a chance to read stories to children while they wait to be seen. Gifts of "gently used" books are welcome.
